Employees must enroll for benefits within 31 days of first becoming eligible, within 31 days of a qualified (IRS) change in family status, or at the next scheduled enrollment period. Detailed information on health plans, including dental and vision, life insurance plans and health and dependent care spending accounts is available online at or by phone from the Your Benefits Resources Customer Care Center (1-866-343-2333).

Employees enroll for benefits online at, or by calling the Customer Care Center (1-866-343-2333). Eligibility and benefit determinations are made in accordance with prevailing plan documents. Additional information and plan brochures are available from Human Resources.

Part-time employees regularly scheduled to work 30 hours per week or more may be eligible for certain benefits on a pro-rated basis. See "Benefits for Part-Time Employees" at the end of this section.

When Changes Occur

When changes occur in your life if you get married, adopt a child, get divorced, retire or change jobs or experience a family crisis, Your Resources Benefits can help you review your current coverage and make appropriate changes to your company and voluntary coverage. To make changes in beneficiaries or in your coverage options, access Your Resources Benefits online at or call the Your Benefits Resources Customer Care Center (1-866-343-2333).
